Advanced Semen Analysis
The foundation of every male fertility assessment — and only useful when performed properly.
Understanding Advanced Semen Analysis
Semen analysis is the single most informative test in male fertility assessment. Yet it is frequently performed poorly, with inadequate abstinence, delayed processing or basic parameters only — producing results that mislead the couple and delay effective treatment.
An advanced semen analysis follows current WHO methodology and assesses considerably more than count and motility. Strict morphology, vitality, leucocyte screening, viscosity, liquefaction time and pH are all evaluated, and DNA fragmentation testing is added where indicated.
The distinction matters clinically. Two men with an identical sperm count can have entirely different fertility potential depending on morphology, vitality and DNA integrity. These are the parameters that explain unexplained infertility and recurrent pregnancy loss, and they are precisely what a basic test misses.
Symptoms
- Inability to conceive after twelve months of trying
- Six months without conception where the partner is over 35
- Recurrent miscarriage or failed assisted reproduction cycles
- Previous abnormal semen result requiring proper reassessment
- Known varicocele, undescended testis or testicular injury
- Before and after fertility treatment or varicocele surgery
- Confirmation of success after vasectomy
- Before chemotherapy or radiotherapy, for sperm banking
If you recognise several of these, an assessment is worthwhile. Early evaluation usually means simpler treatment.
Causes & Risk Factors
Varicocele
Impairs count, motility and morphology through raised scrotal temperature, and is the commonest correctable cause of abnormal parameters.
Infection and inflammation
Raised leucocyte counts indicate infection affecting sperm function and DNA integrity.
Hormonal disorders
Low testosterone, pituitary dysfunction and thyroid disease suppress production.
Heat exposure
Occupational heat, prolonged sitting, tight clothing and hot baths all measurably reduce sperm quality.
Smoking, alcohol and substance use
Each independently reduces count, motility and DNA integrity.
Anabolic steroids and testosterone
Severely suppress production, often for a prolonged period after stopping.
Oxidative stress
Excess reactive oxygen species damage sperm DNA — a common finding in unexplained infertility and recurrent pregnancy loss.
Advanced paternal age
Gradual decline in motility and DNA integrity with age.
Diagnosis at RAMS
We diagnose before we treat. Each step below is chosen for what it will actually change about your management.
Correct preparation
Two to five days of abstinence — shorter or longer intervals distort the result. Recent fever or illness within the previous three months is recorded, as it affects the sample.
Private on-site collection
A dedicated private collection room ensures the sample is processed within the correct time window. Samples produced at home and transported frequently give falsely poor results.
Macroscopic assessment
Volume, viscosity, liquefaction time, pH and appearance, all of which carry diagnostic information.
Microscopic assessment
Concentration, total count, progressive and total motility, and vitality assessed to WHO methodology.
Strict morphology assessment
Detailed evaluation of sperm form using strict criteria, which correlates with fertilisation capacity considerably better than count alone.
Additional testing
Leucocyte quantification, anti-sperm antibody testing, post-ejaculatory urine examination where retrograde ejaculation is suspected, and DNA fragmentation testing where indicated.
Repeat testing
Because sperm parameters fluctuate naturally, an abnormal result is confirmed on a repeat sample before conclusions are drawn.
Treatment Options
Treatment is matched to the cause and to your circumstances. Every option, including doing nothing for now, is explained before you decide.
Interpretation in clinical context
Results are explained alongside examination and hormone findings, rather than handed over as a sheet of numbers. Reference values indicate probability, not a pass or fail.
Identification of correctable causes
Varicocele, infection, hormonal deficiency, medication effects and lifestyle factors are addressed directly.
Lifestyle and occupational modification
Heat avoidance, smoking cessation, alcohol reduction, weight management and antioxidant strategies where appropriate.
Repeat assessment after three months
Because a sperm production cycle takes approximately 74 days, reassessment before three months does not reflect any intervention.
Escalation to further testing
DNA fragmentation, genetic and hormonal testing where parameters are severely abnormal or results do not match the clinical picture.
Sperm banking
Cryopreservation before cancer treatment, before vasectomy, or where parameters are declining.
Coordination with assisted reproduction
Results guide which treatments are appropriate, and are shared with the couple's fertility unit.
Benefits of Treatment
- Testing to current WHO standards, not an abbreviated report
- Strict morphology and vitality that basic tests omit
- Private on-site collection with prompt processing
- Detection of infection and antibody-related causes
- DNA fragmentation testing for unexplained infertility
- Results interpreted by an andrologist, not just reported

Advanced Semen Analysis FAQs
How do I prepare for the test?
Maintain two to five days of abstinence — no more and no less. Avoid alcohol beforehand, and tell us about any fever or illness in the previous three months, since these affect the result.
Can I produce the sample at home?
On-site collection in our private room is strongly preferred, because delay and temperature changes during transport degrade motility and can produce a falsely abnormal result.
Does one abnormal result mean I am infertile?
No. Sperm parameters fluctuate considerably, and illness, stress and abstinence period all influence them. An abnormal result is always confirmed on a repeat sample before any conclusion is reached.
What is DNA fragmentation testing?
It measures damage to the genetic material within sperm. It is particularly useful in unexplained infertility, recurrent miscarriage and failed assisted reproduction, where standard parameters may appear entirely normal.
How soon will results be available?
Standard parameters are usually available the same day. Specialised tests such as DNA fragmentation take longer, and the timeframe is explained when the test is arranged.
Is the test confidential?
Entirely. Collection is in a private room with a discreet entrance, and results are released only to you.
