Premature & Delayed Ejaculation
Among the most common concerns men bring to us — and among the most responsive to treatment.
Understanding Premature & Delayed Ejaculation
Premature ejaculation (PE) is ejaculation that consistently occurs sooner than the man or his partner would wish, accompanied by distress or avoidance of intimacy. Delayed ejaculation is the opposite difficulty: marked delay or inability to ejaculate despite adequate stimulation and desire.
Both conditions are common and both are treatable, yet they are among the least discussed problems in men's health. Men frequently turn first to sprays, unregulated remedies and online advice, which at best delay effective treatment and at worst cause harm — some topical products sold without prescription cause numbness in the partner or contact irritation.
PE is classified as lifelong when it has been present since first sexual experience, or acquired when it develops later. The distinction matters, because acquired PE frequently has an identifiable cause such as erectile dysfunction, prostatitis or thyroid dysfunction that resolves once treated.
Symptoms
- Ejaculation consistently occurring within about a minute of penetration
- Inability to delay ejaculation on nearly all occasions
- Distress, frustration or avoidance of sexual intimacy
- In delayed ejaculation, marked delay or inability to ejaculate despite desire
- Reliance on unusually intense or specific stimulation to reach climax
- Associated erectile difficulty, often as men rush intercourse to avoid losing an erection
If you recognise several of these, an assessment is worthwhile. Early evaluation usually means simpler treatment.
Causes & Risk Factors
Neurobiological factors
Variation in serotonin receptor sensitivity strongly influences ejaculatory latency and largely explains lifelong PE. It is a physiological trait, not a failure of self-control.
Erectile dysfunction
Men with ED often hurry intercourse to make use of an erection before it fades, producing secondary PE. Treating the ED frequently resolves the PE.
Prostatitis and pelvic inflammation
Chronic prostatitis and chronic pelvic pain syndrome are recognised and readily treatable causes of acquired PE.
Thyroid dysfunction
Hyperthyroidism is associated with premature ejaculation, and hypothyroidism with delayed ejaculation. A simple blood test identifies both.
Psychological factors
Performance anxiety, early conditioning towards rapid ejaculation, relationship stress and depression all contribute.
Medication and substances
SSRIs, antipsychotics and alcohol commonly cause delayed ejaculation; opioid use is another frequent contributor.
Neurological and diabetic causes
Diabetic neuropathy, spinal pathology and post-surgical nerve injury more often produce delayed or absent ejaculation.
Diagnosis at RAMS
We diagnose before we treat. Each step below is chosen for what it will actually change about your management.
Structured sexual history
Establishing whether the pattern is lifelong or acquired, situational or global, and estimating intravaginal ejaculatory latency time. This distinction drives the entire treatment plan.
Assessment of erectile function
Because ED and PE frequently coexist, erectile function is always evaluated alongside, and treated first where it is the driver.
Clinical examination
Genital and prostate examination to identify prostatitis, phimosis, frenular tightness or other local contributors.
Targeted blood tests
Thyroid function, testosterone and glucose, with urine and prostatic secretion examination where infection is suspected.
Psychological and relationship assessment
Screening for anxiety, depression and relationship strain, with partner involvement where the couple wishes.
Treatment Options
Treatment is matched to the cause and to your circumstances. Every option, including doing nothing for now, is explained before you decide.
Behavioural techniques
The stop-start and squeeze techniques, pelvic floor training and structured sensate focus exercises build ejaculatory control. These require practice but produce durable results without medication.
Topical anaesthetic agents
Correctly formulated and correctly applied topical agents reduce glans sensitivity while avoiding transfer to the partner — a common problem with unregulated sprays.
Oral pharmacotherapy
Specific serotonergic agents, taken either on demand or daily, reliably extend ejaculatory latency. Selection and dosing are individualised and monitored.
Treating the underlying condition
Managing prostatitis, thyroid disorder or erectile dysfunction frequently resolves acquired PE without any PE-specific treatment at all.
Medication adjustment for delayed ejaculation
Where an antidepressant or other drug is responsible, reviewing it with the prescribing doctor is usually the most effective step.
Counselling and couple therapy
Addressing anxiety, expectation and communication substantially improves outcomes and reduces relapse.
Benefits of Treatment
- Meaningful improvement in ejaculatory control for most men
- Identification of treatable causes such as prostatitis or thyroid disease
- Reduced performance anxiety and improved confidence
- Improved relationship and partner satisfaction
- Avoidance of harmful unregulated products
- Discreet consultation without judgement

Premature & Delayed Ejaculation FAQs
Is premature ejaculation a psychological problem?
Not usually, at least not entirely. Lifelong PE has a strong neurobiological basis relating to serotonin signalling. Anxiety often accompanies and worsens it, but treating the physiology is generally what produces improvement.
Do the sprays and creams sold online work?
Some topical agents are genuinely effective when properly formulated and applied. Unregulated products carry real risks — excessive numbness, transfer to the partner and skin reactions are all common complaints we see.
Will I need medication permanently?
Not necessarily. Many men use medication while learning behavioural techniques, then reduce or stop it. Where PE is caused by another condition, treating that condition often removes the need entirely.
Can premature ejaculation affect fertility?
It rarely prevents conception outright unless ejaculation occurs before penetration. If you are also trying to conceive, mention it — that may change the approach.
Should my partner attend the consultation?
It is welcome but never required. Many men attend alone initially. Where the difficulty is affecting the relationship, partner involvement often improves outcomes.
