Preventive Cardiology
Erectile dysfunction often appears years before a heart problem. That warning is worth acting on.
Understanding Preventive Cardiology
Erectile dysfunction and cardiovascular disease share the same underlying process: damage to the lining of the blood vessels. Because the penile arteries are considerably narrower than the coronary arteries, they become symptomatic earlier — which means erectile dysfunction frequently appears years before angina or a heart attack.
This makes an ED consultation an unusually valuable opportunity. A man who comes in for a sexual health concern is, in effect, presenting an early vascular warning sign, and it would be a missed opportunity to treat the symptom without assessing what it signals.
Preventive cardiology at RAMS is therefore an integrated part of men's health assessment rather than a separate cardiology service. We screen for the modifiable risk factors — blood pressure, lipids, glucose, weight, smoking — and coordinate onward cardiology referral where the findings warrant it.
Symptoms
- Erectile dysfunction, particularly of gradual onset in a man over 40
- Breathlessness on exertion or reduced exercise tolerance
- Chest tightness or discomfort with activity
- Known high blood pressure, diabetes or raised cholesterol
- Central obesity with an increased waist circumference
- Current or previous smoking
- A family history of early heart disease
- Loud snoring with daytime sleepiness, suggesting sleep apnoea
If you recognise several of these, an assessment is worthwhile. Early evaluation usually means simpler treatment.
Causes & Risk Factors
Endothelial dysfunction
Damage to the inner lining of blood vessels is the shared origin of both erectile dysfunction and coronary disease.
Diabetes and insulin resistance
Accelerates vascular damage throughout the body and is strongly associated with both conditions.
Hypertension
Sustained high blood pressure damages arteries; some antihypertensive drugs additionally affect erectile function, which is worth reviewing.
Dyslipidaemia
Raised LDL cholesterol drives atherosclerotic plaque formation in penile and coronary arteries alike.
Smoking
Directly injures the vascular endothelium and is among the strongest modifiable risk factors for both conditions.
Obesity and inactivity
Promote insulin resistance, inflammation, low testosterone and vascular disease simultaneously.
Obstructive sleep apnoea
Independently raises cardiovascular risk and lowers testosterone, and is frequently undiagnosed.
Diagnosis at RAMS
We diagnose before we treat. Each step below is chosen for what it will actually change about your management.
Cardiovascular risk profiling
Blood pressure, waist circumference, BMI and smoking status combined into a formal risk estimate.
Metabolic blood panel
Fasting glucose, HbA1c and a full lipid profile to identify diabetes, prediabetes and dyslipidaemia.
Hormonal assessment
Testosterone, which is independently associated with metabolic syndrome and cardiovascular risk in men.
Baseline ECG
A resting electrocardiogram as part of initial assessment where risk factors are present.
Sleep apnoea screening
Structured questioning for snoring, witnessed apnoea and daytime sleepiness, with onward referral where indicated.
Coordinated cardiology referral
Where findings suggest established disease, timely referral with the full assessment forwarded.
Treatment Options
Treatment is matched to the cause and to your circumstances. Every option, including doing nothing for now, is explained before you decide.
Structured lifestyle intervention
Practical, individualised guidance on physical activity, diet, weight reduction and alcohol — the interventions that improve both cardiovascular and erectile outcomes.
Smoking cessation support
The single highest-value intervention for a man with both erectile dysfunction and vascular risk.
Blood pressure and lipid management
Initiation or coordination of therapy, with attention to selecting agents that do not worsen sexual function where alternatives exist.
Diabetes and prediabetes management
Early identification and coordinated glycaemic control, which improves erectile function alongside cardiovascular risk.
Weight management
Sustained weight reduction improves testosterone, insulin sensitivity, erectile function and cardiac risk together.
Integrated follow-up
Cardiovascular risk reviewed alongside urological and andrological care rather than treated as somebody else's concern.
Benefits of Treatment
- Early detection of vascular disease through an ED consultation
- Identification of undiagnosed diabetes and hypertension
- Lifestyle changes that improve sexual and cardiac health together
- Medication review where drugs are affecting sexual function
- Coordinated referral without duplicated testing
- A whole-health approach rather than symptom-only treatment

Preventive Cardiology FAQs
Why is a urology centre assessing my heart risk?
Because erectile dysfunction is a recognised early marker of vascular disease. Treating the erection without assessing the vessels would mean missing a genuinely useful warning.
Does erectile dysfunction mean I will have a heart attack?
No. It indicates raised risk, not certainty. Identifying that risk early is precisely what allows it to be reduced.
Are ED tablets safe if I have heart disease?
For many men, yes — but they are genuinely dangerous when combined with nitrate medication. This is exactly why a proper cardiovascular review, rather than buying tablets over the counter, matters.
What tests are involved?
Blood pressure, weight and waist measurement, blood tests for glucose, lipids and testosterone, and a baseline ECG. Most of it is completed in a single visit.
Will I be referred to a cardiologist?
Only if the findings warrant it. Where they do, we refer promptly and share the full assessment so nothing is repeated unnecessarily.
