Penile Prosthesis & Reconstruction
A definitive solution when medical treatment for erectile dysfunction has been exhausted.
Understanding Penile Prosthesis & Reconstruction
A penile prosthesis is a device implanted within the erectile bodies of the penis to produce a reliable erection. It is offered to men with severe erectile dysfunction that has not responded to tablets, injections or other medical therapy — and it remains one of the most satisfying procedures in urology, with patient and partner satisfaction rates consistently reported above 90 per cent.
Penile reconstruction covers a broader range of procedures: correction of the curvature and deformity caused by Peyronie's disease, repair after trauma or infection, correction of congenital curvature, and revision of previous unsuccessful surgery.
These are specialised operations. Outcomes depend heavily on correct patient selection, meticulous sterile technique and surgeon experience — which is why they should be undertaken by a surgeon with formal andrology training rather than as an occasional procedure.
Symptoms
- Severe erectile dysfunction unresponsive to tablets and injection therapy
- ED following radical prostate surgery or pelvic radiotherapy
- Long-standing diabetic erectile dysfunction with poor arterial inflow
- Significant penile curvature preventing intercourse
- Painful erections with a palpable plaque, suggesting Peyronie's disease
- Penile shortening or deformity following trauma or infection
- Complications or dissatisfaction after previous penile surgery
If you recognise several of these, an assessment is worthwhile. Early evaluation usually means simpler treatment.
Causes & Risk Factors
Severe vascular erectile dysfunction
Advanced arterial disease or venous leak that does not respond to any medical therapy.
Diabetic erectile dysfunction
Long-standing diabetes causing combined nerve and vessel damage, often unresponsive to oral treatment.
Post-prostatectomy ED
Nerve injury following radical prostate cancer surgery, where erectile function has not recovered after an adequate trial of rehabilitation.
Peyronie's disease
Fibrous plaque formation in the tunica albuginea producing curvature, pain, shortening and often accompanying ED.
Penile trauma or fracture
Injury causing structural damage, scarring or deformity.
Priapism sequelae
Prolonged unresolved priapism leading to fibrosis of the erectile tissue.
Congenital curvature
Structural asymmetry present from birth that interferes with intercourse.
Diagnosis at RAMS
We diagnose before we treat. Each step below is chosen for what it will actually change about your management.
Confirmation of failed medical therapy
Documenting that oral agents have been correctly dosed and adequately trialled, and that injection therapy has been attempted where appropriate. Surgery is never a first step.
Penile Doppler ultrasound
Assessing arterial inflow, venous competence and plaque location, and mapping the extent of fibrosis.
Curvature assessment
Objective measurement of curvature using photographs or an induced erection test, with documentation of plaque size and position.
General fitness and infection screening
Glycaemic control, urine culture and skin assessment. Well-controlled diabetes and a sterile field markedly reduce implant infection risk.
Counselling and expectation setting
A detailed discussion of what an implant does and does not change — including that it does not alter sensation, orgasm, or penile length beyond what is already present.
Treatment Options
Treatment is matched to the cause and to your circumstances. Every option, including doing nothing for now, is explained before you decide.
Inflatable penile prosthesis
A multi-component device allowing the man to inflate and deflate the erection at will, giving the most natural appearance in both the erect and flaccid state.
Malleable (semi-rigid) prosthesis
A simpler, bendable device that is easier to operate, has fewer mechanical parts, and suits men with limited manual dexterity.
Peyronie's plaque surgery
Plication or plaque incision with grafting to straighten the penis, selected according to curvature severity and erectile status.
Combined implant and straightening
Where Peyronie's disease coexists with severe ED, curvature correction and prosthesis implantation are performed in the same operation.
Reconstruction after trauma or infection
Repair of structural damage, scar correction and, where necessary, staged reconstruction.
Revision surgery
Correction of complications or unsatisfactory results from previous procedures performed elsewhere.
Benefits of Treatment
- A reliable erection available at any time, without tablets or injections
- Very high long-term patient and partner satisfaction
- Sensation, orgasm and ejaculation are preserved
- The device is entirely concealed within the body
- Correction of curvature that prevents intercourse
- A one-time definitive solution rather than ongoing medication

Penile Prosthesis & Reconstruction FAQs
Will the implant be visible?
No. The entire device sits within the body, and in the flaccid state the penis appears normal. An inflatable device gives the most natural appearance.
Will I still feel sensation and have orgasms?
Yes. A prosthesis restores rigidity only. Penile sensation, the ability to orgasm and ejaculation are all unaffected, because the nerves responsible are not disturbed.
How long does an implant last?
Modern devices are durable, with the majority functioning well beyond ten years. Mechanical failure can occur eventually and the device can be replaced if needed.
Is the surgery painful, and how long is recovery?
There is discomfort and swelling for the first one to two weeks, controlled with medication. Most men return to routine activity within two to four weeks, and the device is generally used from around six weeks after surgery.
What are the main risks?
Infection is the most important, which is why glycaemic control and sterile technique matter so much. Other risks include mechanical failure and, rarely, erosion. These are discussed fully before you decide.
Does an implant make the penis longer?
No, and any surgeon promising this should be treated with caution. An implant restores rigidity to the existing anatomy. Men with prior Peyronie's shortening should have realistic expectations discussed in advance.
