Overview

Understanding Testicular Problems

Testicular problems range from the genuinely urgent to the entirely benign, and telling them apart quickly matters a great deal. Testicular torsion, in which the testis twists on its blood supply, causes permanent loss of the testis within hours if it is not operated on.

For that reason, sudden severe testicular pain — particularly in adolescents and young men — should be treated as an emergency and assessed immediately, not the following day. Our centre operates 24 hours precisely so this is possible in Hanamkonda and the surrounding Warangal district.

Other testicular conditions are less urgent but still deserve proper evaluation. Varicocele is the commonest correctable cause of male infertility; a painless lump may be an early testicular tumour, which is highly curable when caught early; and chronic scrotal discomfort has a number of causes that respond well to targeted treatment.

Signs to Watch For

Symptoms

  • Sudden severe testicular pain, often with nausea or vomiting — a surgical emergency
  • A painless lump or firmness within the testis — always requires urgent assessment
  • Gradual scrotal swelling or a feeling of heaviness
  • A soft 'bag of worms' swelling above the testis, typical of varicocele
  • Aching or dragging discomfort, often worse after standing or exertion
  • Redness, warmth and tenderness with fever, suggesting infection
  • One testis noticeably smaller or softer than the other
  • An empty scrotum on one or both sides, indicating undescended testis

If you recognise several of these, an assessment is worthwhile. Early evaluation usually means simpler treatment.

Why It Happens

Causes & Risk Factors

Testicular torsion

Twisting of the spermatic cord cutting off blood supply. It is a true emergency: salvage rates fall sharply after six hours.

Varicocele

Enlargement of the draining veins, causing aching, testicular shrinkage and impaired sperm production.

Hydrocele

Fluid collection around the testis causing painless, gradually increasing swelling.

Epididymo-orchitis

Infection or inflammation of the testis and epididymis, causing pain, swelling and often fever.

Testicular tumour

Typically presents as a painless firm lump. Testicular cancer is among the most curable of all cancers when detected early, which is why any lump needs prompt assessment.

Epididymal cyst or spermatocele

Benign fluid-filled swellings that usually need only reassurance and observation.

Undescended testis

A testis that failed to descend in childhood, carrying reduced fertility and an increased tumour risk lifelong.

Chronic scrotal pain

Persistent discomfort following infection, surgery, hernia repair or vasectomy, or arising without clear cause.

How We Investigate

Diagnosis at RAMS

We diagnose before we treat. Each step below is chosen for what it will actually change about your management.

1

Urgent clinical examination

Immediate assessment where torsion is suspected. Certain clinical signs strongly suggest torsion, and where suspicion is high, surgical exploration proceeds without waiting for imaging.

2

Scrotal Doppler ultrasound

The key investigation, showing blood flow, testicular structure, varicocele grade, hydrocele and any solid mass.

3

Urine analysis and culture

To confirm or exclude infection as the cause of pain and swelling.

4

Tumour markers

AFP, beta-hCG and LDH where a solid testicular mass is identified.

5

Semen analysis and hormone profile

Where varicocele, undescended testis or testicular atrophy raises fertility concerns.

6

Cross-sectional imaging

CT staging where a testicular tumour is confirmed.

Your Options

Treatment Options

Treatment is matched to the cause and to your circumstances. Every option, including doing nothing for now, is explained before you decide.

1

Emergency scrotal exploration

Immediate surgery for suspected torsion, untwisting the cord and fixing both testes. Time is the single most important factor in saving the testis.

2

Microsurgical varicocelectomy

Microscope-assisted ligation of the abnormal veins, preserving the artery and lymphatics, with lower recurrence and hydrocele rates than conventional surgery.

3

Hydrocelectomy

Day-care surgical correction of symptomatic hydrocele.

4

Antibiotic therapy for epididymo-orchitis

Targeted antibiotics guided by culture, with supportive measures and follow-up to confirm resolution.

5

Radical orchidectomy and oncological care

Surgical removal via an inguinal approach where a tumour is confirmed, with staging and onward oncology coordination. Sperm banking is discussed beforehand where fertility is a consideration.

6

Orchidopexy

Surgical placement of an undescended testis into the scrotum, with long-term follow-up for fertility and tumour surveillance.

7

Chronic scrotal pain management

A stepwise approach including infection treatment, nerve-directed therapy, and microsurgical denervation in carefully selected refractory cases.

Questions Answered

Testicular Problems FAQs

How quickly must testicular torsion be treated?

Within hours. Salvage rates are high when surgery occurs within six hours of onset and fall sharply thereafter. If you have sudden severe testicular pain, come immediately — at any hour.

Is a painless lump less serious than a painful one?

No, and this misconception costs lives. Testicular tumours are typically painless. Any firm lump within the testis needs assessment promptly, regardless of whether it hurts.

Does every varicocele need surgery?

No. Many are asymptomatic and need only monitoring. Surgery is considered where there is pain, testicular shrinkage, or impaired semen parameters in a man seeking fertility.

Will removing one testis affect my fertility or hormones?

Most men retain normal hormone levels and fertility with one healthy testis. Where the remaining testis is abnormal, or where chemotherapy is planned, sperm banking is discussed beforehand.

Can I check myself at home?

Yes, and it is worth doing. A monthly self-examination after a warm shower helps you notice changes early. Report any new lump, firmness or change in size or shape.
